What is the difference between Fraxel treatments and Erbium resurfacing?

While both treatments are FDA approved for the treatment of wrinkles and acne scars, they work in a very different manner.

Erbium resurfacing works by removing the surface layer of the skin (the epidermis). As the epidermis regenerates, new cells develop without the memory of the wrinkles or acne scars that were previously present. Because the laser energy penetrates into the dermis, collagen remodeling is seen in the epidermis (upper layer) as well as the dermis (lower layer). Three treatments are typically required and redness/peeling for Erbium resurfacing is typically between 5 and 7 days.

The Fraxel lasers user “fractionated” technology. This means that the laser beam is split into many thousands of microscopic beams. These microscopic beams penetrate the skin and cause the tissue affected to fall out and be regenerated while the tissue surrounding the small beam remains intact. Several Fraxel treatments are normally suggested and swelling and redness are typically present for 5-7 days.

What exfoliation treatments does Celibre offer?

Tretinoin – Tretinoin is the acid form of Vitamin A. It is one of the most commonly used products for increasing cell turnover (exfoliation). It is used in many bleaching products in combination with other acids/ingredients. Tretinoin may be used in acne and/or rejuvenation programs.

Glycolc Acid – Glycolic acid is the smallest alpha hydroxyl acid. It is the most commonly used ingredient in over the counter products for exfoliation. It works by breaking down the cell wall between dead skin cells, making it easier for these cells to fall off. Because of its diminutive size the glycolic acid molecule is extremely effective at penetrating the skin. Glycolic acid may be used in the products for our acne and/or facial rejuvenation programs.

Salicylic Acid – Salicylic acid is similar to glycolic acid, but a slightly larger molecule. Like glycolic acid, it is used to treat acne and rejuvenation patients because it increases cell turnover by breaking down the cell walls of dead skin cells, causing them to flake off more easily. Salicylic acid may be used in the products for our acne and/or facial rejuvenation programs.

What does the DUSA Blu -U acne lamp destroy?

If you are an acne patient you may already know this, but for those of you that are not, the DUSA Blu-U lamp is one component of the acne laser program. It is not a laser but a very bright lamp that uses high energy blue light to kill acne bacteria. It is a fast and easy treatment to help clear up mild to severe breakouts.

How can I get immediate results for my condition?

Did you know that injectable treatments offer some of the most immediate results available at Celibre? Using injectable treatments such as Botox, Restylane and/or Juvederm is a sure fire way to achieve near immediate results for facial rejuvenation. In general, the results for Botox injections are realized within several days. Laser Hair Removal – What You Need to Know

In trying to determine which laser hair removal provider is right for you, it’s important to know which type of lasers are safe and effective for your skin type. Let’s start by defining the skin types.

There are six skin types, I – VI

  • Type I skin is of Caucasian descent, is very fair and is typically Northern European or Scandinavian such as Swedish or Irish.
  • Skin Type II is also fair and is normally associated with Caucasian skin types of European descent. English, German, Russian etc. are usually skin type II.
  • Skin type III includes Caucasians of Mediterranean descent and some lighter skinned Hispanics. Many Italians, Spanish, Greeks and light skinned Hispanics from South American are skin type III.
  • Skin Type IV is typically medium skinned Hispanics, Asians, and Persians/Middle Easterners.
  • Skin Type V is typically light skinned North Africans or African Americans.
  • Skin Type VI is associated with dark skinned Africans.

Lasers for Laser Hair Removal

Now that you know something about your skin type, below are a few laser choices for laser hair removal. There are several fairly well accepted types of Class IV medical lasers for hair removal:

  • Diode Laser (Lumenis Lightsheer)
  • Alexandrite Laser (Many Manufacturer’s)
  • Long Pulsed Nd:Yg Laser (Many Manufacturer’s)

The most common hair removal laser, the Diode laser, is the Lumenis Lightsheer. Many consider it the gold standard for laser hair removal as it has the widest use among practitioners. The Lumenis Lightsheer is ideal for skin types I-IV and can be used on higher skin types (according to the manufacturer) if certain models of this equipment are used.

The Alexandrite Laser is less commonly used laser but very effective for hair removal in skin types I-III. Generally it is not used above skin type III because there is a risk of hyperpigmentation (a change in skin color) and or blistering. Although these are potential side effects for any laser treatment, the chance of the side effects happening when using an Alexandrite laser for hair removal above skin type III increases greatly. There are several brands of Alexandrite lasers but the Cynosure Apogee is a widely used model.

The long pulsed Nd:Yg is used for all skin types and is the most common laser used for skin types III, IV, V and VI. The reason is that the physical properties of this laser allow it to penetrate more deeply than the first two lasers. By penetrating more deeply into the skin, this laser is able to bypass the upper layers of skin that contain melanin, the body’s natural coloring. Melanin can compete with the coloring in the hair for the laser’s energy and in darker skinned patients, this laser allows the practitioner to lessen the risk that the laser energy will be absorbed at the surface of the skin (potentially leading to a side effect) rather than at the root of the hair follicle where the energy should be absorbed. There are many long pulsed Nd:Yg lasers to choose from. One common model of long pulsed Nd:Yg is the Sciton Profile.

Before choosing a laser hair removal provider because of their location or their price, consider choosing one based on how well the technology will work for your skin type and how safe it is for you.

Birthmark Removal Before & After Pictures

One of the more specialized treatments we perform at Celibre Medical is birthmark removal. While not all birthmarks can be removed with laser treatments, many can. Below is an example of one type of birthmark we can successfully treat, the cafe-au-lait.

Pigmented birthmarks such as café-au-lait birthmarks are brown in color. These birthmarks typically respond well to laser treatments and clearance can usually be achieved between 3 – 6 treatments. Unfortunately, this type of birthmark has a reputation of returning (50%) within a few years after successful treatment. However, the cause for reoccurrence is usually associated with sun exposure. Other types of brown birthmarks are caused by a higher then normal concentration of pigment in the skin. This pigment is the natural color of our skin but becomes darker because of the abnormally high concentration of it in a birthmark. Many pigmented (brown) birthmarks are quite responsive to Q-Switched lasers but the realistic goal oftentimes is improvement rather than complete resolution.

Hairy Nevi – Not a Candidate for Laser Birthmark Removal

There are a few types of pigmented birthmarks that we do not treat with lasers because some may be better suited for surgical intervention or, in some cases, may have a predisposition for becoming pre-cancerous.

Port Wine Stain Birthmark Removal

Another type of vascular (red) birthmark is referred to as a Port Wine Stain. These types of birthmarks are red in color due to the presence of vascularity (blood supply). This condition usually is associated with a cosmetic concern since they are difficult to camouflage and occur most often on the face and neck. Pulsed Dye laser treatments are an excellent option for these types of birthmarks, and this technology is recognized as the gold standard for their effective removal. The expectation should be several treatments – sometimes as many as ten.

How are acne scars treated with lasers?

We often receive this question in our e-mail box after users on our website browse though before and after pictures of acne scars. Although it may seem like a simple question, there are actually many treatment options based on a patient’s condition and lifestyle.

Celibre is able to treat red and brown colored acne scars as well and textured acne scars. Some treatment plans allow for both the long term prevention of acne and the treatment of acne scars simultaneously. Generally, it is very difficult if not impossible to determine what kind of treatments we would suggest for a potential patient until we see them up close. We need to understand their goals and then design a program that fits their situation and this is done during the initial consultation.

Textured Acne Scars. Textured acne scars have two different treatment programs available. One of these solutions (ablative resurfacing) comes with what we call “social downtime” which is approximately 5-7 days of redness swelling and peeling and the other comes with no downtime (non-ablative resurfacing). No downtime laser resurfacing is very effective at developing new collagen for shallow surface scars, especially in situations where the patient still has active acne. Ablative (downtime) laser resurfacing is a more aggressive acne scar laser treatment program This program is ideally suited for deeper pockmarked or ice-pick scars, and results are generally more dramatic than no downtime laser resurfacing. The ablative laser resurfacing is a program that requires the patient to be home for several days after the treatment for recovery.

Brown Acne Scars. Pigmented (or dark spots) acne scar removal is entirely different than textured acne scar removal treatments. Removing dark acne scars may involve laser procedures and/or topical skin care products. Our first laser option involves q-switched lasers, which we use to treat brown acne scars. Q-switched lasers work by heating up the pigment in the skin to the point where it fragments. It can then be removed by the body’s lymphatic system leaving a patient with a more even skin tone. Celibre Medical also uses two separate depigmentation programs for brown acne scars. These products prevent the body from making the pigment that causes brown acne scars.

Red Acne Scars. Lasers can also be used to treat red acne scars. A pulsed dye laser is used to remove all kinds of redness associated with vascular conditions including acne and Rosacea . It is used to remove small red acne scars by closing the microscopic capillaries associated with the redness.

Questions for Your Laser Hair Removal Facility

Below is a list of questions we feel are important to ask to make sure that you have a good experience with laser hair removal.

How many laser hair removal treatments have you performed?

In California, there are no official regulations regarding laser hair removal training for nurses and physicians. For this reason, it’s possible that the person performing your laser hair removal treatment may not have any formal training with a laser. For this reason, it’s important to know how much experience your particular nurse or physician has before proceeding with laser hair removal. Laser hair removal practitioners with greater experience will generally provide treatments that will allow you to get quicker laser hair removal results because their experience allows them greater discretion when deciding on the starting energy for your laser hair removal treatments.

What kind of laser is being used for my laser hair removal?

This is a very, very important question. Depending on your skin and hair type there are some lasers that will work better than others for laser hair removal and there are some lasers that don’t work at all. This can cause you to have a bad experience with laser hair removal. In situations when the wrong laser is used, it can lead to either very little hair being permanently removed or many more treatments than necessary with the right laser.

How many laser hair removal treatments will be required?

The number of laser hair removal treatments required is directly related to how good the practitioner is and how well suited the laser is for your skin and hair type (see points 1 and 2 above). If an experienced practitioner is using the correct laser for your skin type, normally excellent permanent laser hair removal results occur within 6-8 treatments.

What percentage of hair will I lose?

In our practice, we are able to achieve between 70% and 95% hair loss for our patients. The amount of hair loss depends on the type of hair (dark or light coarse or fine) and the hair that will disappear first are the coarsest, darkest most visible and problematic hairs. In most cases the ones that remain are the finer thinner hairs with little color. They is a very small percentage of the population (less than 5%) that do not fit into this category and they are non-responsive for unknown reasons.

How often do I go in for laser hair removal treatments?

The time between laser hair removal treatments varies between a 4 and 12 weeks with the average being 6-8 weeks. It depends on the area of the body being treated as each area has a different hair growth cycle.

How often do side effects occur?

At Celibre, we know that about 3 in every 1,000 laser treatments results in an unforeseen outcome. We do not know what the industry average is, but we do know that using the wrong laser and having an inexperienced practitioner can increase this rate.

Juvederm is Now Available at Celibre!

Celibre is happy to announce the launch of Juvederm from Allergan – the makers of Botox. Medical professionals use Juvederm, one of the newest dermal fillers, to decrease wrinkles and folds by adding volume and raising the level of the skin. They commonly employ Juvederm to address laugh lines, which are lines running between the side of the nose and the side of the mouth.

Injecting Juvederm below the skin’s surface reduces or eliminates the appearance of these lines. Medical professionals also use Juvederm to address smoker’s lines above the lips and treat other deep wrinkles or folds.

Hyaluronic acid, a naturally occurring compound in our skin, forms the basis of Juvederm. The property that makes hyaluronic acid unique is that it has the ability to absorb 1,000 times it weight in water. In fact, the reason baby’s skin is so soft and smooth is that it contains very high levels of hyaluronic acid.

Photodynamic Therapy With Aminolevulinic Acid (ALA)

Since 2004, Celibre Medical has been using a treatment called Photodynamic Therapy with Aminolevulinic Acid (ALA/PDT). This treatment enhances laser and light treatments that have already been proven successful by adding a light sensitive antibiotic to treat acne, sebaceous hyperplasia and Rosacea/facial redness.

What skin conditions are treated with ALA/PDT?

At Celibre, we have been using ALA/PDT for the treatment of cystic/nodular acne as well as acne vulgaris. This treatment is a safe option for those patients considering an alternative to Accutane or with those that did not respond well to Accutane or traditional non-ablative lasers.

ALA/PDT is also routinely used for the treatment of sebaceous hyperplasia which is a common skin condition that appears as raised, fleshy bumps on the skin. These lesions usually appear on the oiliest parts of the face. Because ALA/PDT is absorbed preferentially by the cells within these oil glands, this treatment is quite effective at shrinking them. For those patients with facial redness and Rosacea, ALA/PDT is a good option in addition to our traditional and effective medical lasers. Although there is no cure for Rosacea, lasers and/or ALA with PDT are excellent options for controlling and improving this condition. By targeted the small blood vessels that contribute to Rosacea, we can decrease redness and improve the overall skin texture.

How will I know if ALA/PDT is a good option for me?

Although ALA/PDT is safe for all skin types, there are a few issues that may prevent you from using this treatment option. An evaluation by our medical team will help to determine this. ALA/PDT also involves downtime away from work, school, and other activities due to the strict sun avoidance during the first 48 hours as well as diligent sun precautions for several days thereafter.

What expectations should I have with ALA/PDT for the treatment of these skin conditions?

We set the expectation that a minimum of two treatments spaced 2-6 weeks apart is usually necessary. Many require 3 or 4 treatments; very few will need beyond this. Although our medical lasers are also able to address acne, Rosacea, and Sebaceous Hyperplasia, not every treatment plan will fit your condition, goals, and lifestyle. Knowing that there are additional options such as ALA/PDT has been of much value to our patients that are struggling with their particular situation. Whether it is acne, sebaceous hyperplasia, or facial redness, ALA with PDT has provided our patients with good, consistent outcomes.
